Overview

Multnomah County Department of County Human Services (DCHS), Aging, Disability & Veterans Services invites applications for Case Manager Senior. DCHS provides support for well-being throughout life and believe we all benefit when everyone can reach their potential and can fully contribute to our communities. The eligible list created from this recruitment may be used to fill future regular, limited duration, temporary, on-call, full or part-time assignments within the department.

We are seeking talents with a heart of a public servant to help promote and provide older adults and persons with disabilities with services and programs that encourage independence and dignity. Case Manager Seniors perform highly complex case management duties, in order to determine initial eligibility for services and benefits for a broad spectrum of Federal and State programs including Medicaid programs, nursing facility services, state plan personal care services, and Supplemental Nutrition Assistance Program (SNAP).

Main responsibilities for this position include:

  • In-depth screenings of potential clients

  • Initial and ongoing eligibility determination

  • Assessments of functional abilities

  • Complex financial eligibility determination

  • Development and implementation of detailed care plans

  • Interpret and apply state/federal/county rules and guidelines

  • Relocation assistance

The successful candidate will demonstrate:

  • Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values

  • Knowledge of public benefit programs including state, regional/county-based programs, and providers of services to older adults and persons with disabilities

  • Ability to interpret/apply federal, state, and local laws, rules, regulations, policies and procedures related to program scope and operations

  • Experience and/or knowledge of conducting assessments and determining eligibility for service programs

  • Ability to communicate clearly and concisely, both orally and in writing

  • Ability to exercise discretion and judgement in the research, analysis, and interpretation of all work-related assignments

  • Ability to problem-solve in a fast-paced work environment

Minimum Qualifications/Transferable Skills*:

  • Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values

  • Education/Training: Associate's degree or equivalent practical experience

  • Experience: Four (4) years of case management experience with a minimum of one year of experience at the full performance/journey case management level

  • Must have a valid driver's license

  • Must pass a criminal background check
