Are you interested in supporting at risk youth in your area? Southwest Key Programs is opening new youth services programs in Florida. With offices in Panama City, Pensacola and Tallahassee. We are looking for Case Managers to work with the youth and their families. The Case Manager is responsible for the orientation and intake, assessment and discharge process with a youth and family. They complete the C-PACT, facilitate the completion of the YES plans with the families and the full wraparound case management process. The Case Manager is responsible for collection and review of all assessment documents to be shared with team during case staffing's. The Case Manager maintains a flexible, organized and efficient work schedule and is subject to work more than a normal work day and be on-call to facilitate program services.
Case Managers shall possess a Bachelor’s degree, or an Associate’s degree with two years of experience working with youth or four years of experience working with youth. A combination of education and appropriate experience shall meet this qualification. Degrees must be conferred from an accredited college or university.
